What it means

The engine computer has been adding extra fuel to compensate for too much air or too little fuel, and has reached its correction limit. Bank 1 is the side with cylinder 1; on four-cylinder engines it is the whole engine.

Hesitation or stumbling on acceleration.
Occasional hard starting.
Check engine light with no obvious symptoms in some cases.

Dirty mass airflow sensor reading less air than actual.
Low fuel pressure from a weak pump or clogged filter.
Partially clogged injectors.
Exhaust leak ahead of the upstream oxygen sensor.
Cracked intake duct between the air filter and throttle body.

Inspect vacuum hoses and the main intake duct for cracks or hissing.
Clean the mass airflow sensor and check the air filter.
Have a smoke test done if no leak is visible.
Measure fuel pressure against the specification.
Check the exhaust for leaks ahead of the oxygen sensor.
Clear the code and monitor fuel trim values with a scan tool after the repair.

Both banks running lean points to a shared cause such as the airflow sensor, fuel pressure or an intake manifold leak.
